Below is a sample survey I created requesting information from users regarding an older system in preparation to create training materials for the new version of the system.

Surveys such as this one are helpful in allowing users to feel like they have a say in their training materials and showing them that their opinions are valuable. It is also critical to evaluate training materials on a regular basis to ensure their efficiency, usability, and quality. The first step in creating new materials is always to evaluate the older materials to determine what should be carried over and what needs to be updated and improved upon.
